multiple selections on a form

GOAL: i'd like to be able to have my users make multiple
selections (via a check mark or "dot" button) and the
click a button (submit or whatever) and have only those
selections carry over to a form page that can be
submitted.

I have one form for people to fill out that is just more
of a questionaire, then i want another form where users
can select only certain builders to get bids from.

The 'radio ("dot")' button only allows one selection in a 'group' the Checkbox
allows multiple selections within a group

i.e. the name of the field might be "email" and the values of the check boxes
could be "james, john, matthew"

Same idea with the radio button, but only one option is selectable.

Those elements are already in the form, what do you mean by 'carry over to the
form'.?
