Multiple forms

Hello,
I would like to create an application with three forms. I've looked for
solution on the Internet, but hopelessly :-((
I've just found an article describing my problem

without any practical example! Could anyone help me?

What's impractical about the sample? It has 3 Forms and you can navigate
between them. At the top of the article is a download for the code.
