Modifying custom toolbar

I created a toolbar a while ago and now I want to update it. I have added a
new command, "Choose Form". However, I wish to assign to a template I
created and change the command name. I have seen this done somewhere, but
don't know how to do it. The command is on my custom toolbar and when
clicked asks me to choos the form. I want to automate for user.

WinXP, Outlook 2003.

Right click toolbar -> customize.. -> right click choose form -> assign
hyperlink and change name:..?
 
Thanks for the response, but it was no help. Right-click on "Choose Form"
does nothing.

Is something not set right?

First you have to choose customize. Right click on toolbar-choose
customize..., or view -> toolbars -> customize..., after that right
click on choose form...

and then I mean right click choose form on the toolbar..
 
That's exactly what I've tried. The "Choose Form" selection in the
"Customize" box (View/Toolbar/Customize) does not right click.

Also, the custom toolbar I have will not allow right click either.

Not in the box, but on the toolbar. Drag a new icon to a toolbar and try
to right click that icon..? But don't close the 'customize window!
