Modify Existing Field Names in Contact Form

Is it possible to modify the existing fields in the Contact Form? We are a
Civil Engineering firm that deals with the local Municipalities departments,
such as Planning, Building, Public Works, Fire, water departments and
telephone companies, etc.

I would like to use the Phone Number Area to situate these telephone numbers.

Is it possible to change the existing fields that we don't use, such as
"Pager", "Radio", "Telex" to say "Planning", "Building", "Public Works", etc.
-- OR--conversely, would it be possible to add fields that would accept my
required heading names into that area?
 
No, that's not possible.

While that article is a good tutorial on creating custom forms, it does not answer the original poster's question correctly. It is not, in fact, possible to change the names of any existing fields, nor is it possible to populate the Phone Number drop-down list with custom fields.
