Missing "Table" button

I used to have a button on my toolbars that looked like a small table, that
when I clicked and held the L mouse button, opened a grid that allowed me to
pick rows and columns for a table.

Somehow I have lost this - the button I have now just opens the "Insert
Table" dialog box. Where has my automatic "Table" button gone ? I can't find
it on the Commands for Tables in the customise menu....

view | Toolbars | Customize | Commands | Table | Drag and drop the "insert
table" button onto the toolbar

You currently have the TABLE INSERT button on your toolbar not the INSERT
TABLE button!

In All Commands in Tools | Customize, the one you want is
TableInsertGeneral.
