Merging with Word

I have exported a Form that I created in Outlook 2000 into Outlook 2003.
When I try to merge an Outllok record with Word, the fields that I have
created in my form do not appear as a potential merge field. After taking a
look into Outlook, the customized merge fields that I created are no longer
in "User-defined fields in folder" but are in another group of fields.

Do I need to transfer the fields from this other folder into "User-defined
fields in folder" and if so how?
