Look in tools > templates and add-ins. It is usually called WFWRD2K2.DOT.
Then delete the file from the Word or Office startup folder. the former is
defined in tools > options > file locations > startup, the latter is
C:\Program Files\Microsoft Office\OFFICE11\STARTUP (for office 2003) -
Office10 for Office XP and Office for Office 2000.
