Lost Min/Max buttons and close button on my form

Hi,

Not sure how I managed to do this but I have lost the
min/max buttons and the close button on a form in my
database.

In the properties sheet for min/max both are enabled and
the close button option is set to yes.

Any ideas how I can fix this?
