Add-in it won't make a difference as it would be your own customized Toolbar
then.
Next approach;
-Disable the add-in through Tools-> Options-> tab Other-> button Advanced
Options...-> button COM Add-Ins/Add-In Manager
-close Outlook and locate outcmd.dat and extend.dat and rename them to .old
-start Outlook again and enable the add-in again
-you might need to restart Outlook for changes to take effect.

I would but Lookout is no longer listed as an available toolbar. Adding the
name "Lookout" as a new toolbar yields an empty toolbar.

After renaming outcmd.dat, the Lookout toolbar disappeared completly. I
re-installed Lookout, but still no toolbar (I did not remove Lookout
first).
Any more ideas?

If that doesn't work reset the entire toolbar by renaming outcmd.dat to
.old

I have been using lookout but recently, the lookout taskbar is empty.
The
taskbar is displayed but has no icons. I have removed Lookout and
reinstalled and I have run the repair for Office/Outlook with no luck.
Any ideas?
