location dialog box with contacts

In the past I have used outlook express, but I could not
customize the company signature to default, so I switched
to microsoft outlook. The problem I am having is that
when I enter the contact information for phone numbers a
dialog box comes asking for area code, access code, etc.
It won't allow me to close this box and if I do 'control
alt delete', it shows the info has sometimes been entered
in the contact list, but when I start to type the e-mail
address in TO: for e-mail purposes, it forgets the name
is there and I have to type the e-mail address in. It
appears to want to connect me via phone, and besides the
fact that I do not want connection, we have a cable
provider, not dial up. It is making me crazy...any help
out there?
