Linking Tasks

I would like to create a form (possibly in Word or Adobe?) and then link it
to a database (possibly Access?) where information on the form can be
automatically updated into the database. Example: Form includes name, date
of birth, zip code, etc. I would like to have this information automatically
transferred into a database where I can later do specific
quaries/sort/searches.

Which would be the best software(s) to use for sucha function?

I'm not sure why you wouldn't want to use an Access form but you could use
something like InfoPath. Each user requires a license.
