Linking Excel and Word

Hi, I have a word document that nurses at the hosptial I work out will fill out and we would like to be able to link the word document to excel so that each box they fill in would be sent to the excel document to help us to be able to track and graph the results. Does this sound possible to anyone? I know this is a litle vauge, so if you need more details, just email me. Thank you very much.
 
If you create a form in Word, you can set it to save the Data only
(Tools>Options, Save tab, 'Save Data only for forms').

This will create a text file when you save it. You can import the text
files to Excel, and store them in a list for analysis.
