I am not sure about "automatic" but if you turn on the Forms toolbar (Word
2000, 2002, 2003), click the Lock icon at the extreme right side, the Tab key
on the keyboard will move you from field to field, and will not allow editing
anything else but the contents of the fields.
Can also be done from Tools - Protection - Form (allows setting password to
unprotect)

Put the formfields in the cells of a table with fixed row height.
