Your HR department should have something on hand. There are so many laws
that cover terminations that a freebie template releasing someone needs to
be reviewed by someone familiar with the laws in your state.
(My best friend is in HR and I've learned a lot about the nit-picky laws on
these things. And HIPAA - oh my! Makes me glad I am *not* in her field.)
Why not just write one that you change the date and the names when needed?
