LDAP

Are we to assume you do not use an LDAP?
If not, remove it as your address book service and use the Outlook Address
Book Service instead.
Method depends on your Outlook version which you did not post.
 
Thank you for your help so far. Excuse my ignorance but I thought I was using
Outlook Address Book Service. I am using Office 2003. Please advise how to
remove LDAP and use Outlook Address Book instead.

Well if Outlook is asking for your LDAP location, then you've obviously
configured it to use an LDAP instead of the Outlook Address Book.
Remove it then add the Outlook Address Book using these instructions:
