Labels in a Form

I have a form where the user needs to enter a score for a category. I would
like to provide details (criteria) as to each category so the user can enter
a score.

For example:

Category A:
Blah blah blah blah blah - 5 points
Blah blah blah blah blah - 4 points
Blah blah blah blah blah - 3 points
Blah blah blah blah blah - 2 points
Blah blah blah blah blah - 1 points

The textbox to enter the score for Catg A would go here ______

Category B:
Blah blah blah blah blah - 5 points
Blah blah blah blah blah - 4 points
Blah blah blah blah blah - 3 points
Blah blah blah blah blah - 2 points
Blah blah blah blah blah - 1 points

The textbox to enter the score for Catg B would go here ______

Category C:
Blah blah blah blah blah - 5 points
Blah blah blah blah blah - 4 points
Blah blah blah blah blah - 3 points
Blah blah blah blah blah - 2 points
Blah blah blah blah blah - 1 points

The textbox to enter the score for Catg C would go here ______

Is the best way to do all the text above as a unassociated label, it came up
with an error, but I told it to ignore it. Is there a better way???

I also need to add the 3 scores into a total score for the item... Can
someone please provide some help with that

Unless "Blah blah blah blah blah" is extremely long, my impulse would be to
use combo boxes. The user selects the appropriate "Blah blah blah blah blah"
from the combo box, and there's no need for the text box.

Another alternative would be to use a single select list box, so that all
five "Blah blah blah blah blah" lines are seen at once.

A third alternative would be to use an Option Group, with each "Blah blah
blah blah blah" corresponding to a radio button in the group.
