Issue with Word Form Checkboxes

I was forwarded a Word form by email that has checkboxes after questions.
When the originator looks at the form, the Yes No checkboxes have rectangles
around them. When I open the form, my checkboxes look like the example below.
We both are using Word 2003. Any ideas as to why the difference? Thanks,

• Are you at least 18 years old?(must be ≥ 18 years old at enrollment)

• Do you have documented Ulcerative Colitis that has maintained remission

Does the form constitute the body of an e-mail message or was it sent as an
attachment? If the former, get it resent as an attachment. If the latter,
then note that this is a text based forum and your formatting does not carry
so we have no idea what the problem may be from the sample.

Sorry for the confusion. It was sent as an attachment. The checkboxes appear
as an "I" in front of "Yes" or "No" instead of a rectangle.

Do you both have the same font installed that was used for the check box?

Thanks Graham! I sent it to your website.

I have returned it. It appears that the document was set up on a PC
configured to use a right to left language and includes some Arabic
formatting and the offending characters are from the Arabic extended
character set.
