Is there a Crest Transfer form template available for download?

As a private dealer in stocks and shares from time to time, I frequently need
to use Crest Transfer forms. If there was an on-line template into which I
could type the relevant details on my computer keyboard this would be easier
and less time consuming than filling in the forms by hand.
Can anyone help?

It might be possible to construct such a form using Microsoft Access (the
subject of this newsgroup), but it would be sort of like cracking nuts with a
sledgehammer... I have no idea what "Crest" forms are like, but I think you
should be asking elsewhere for such forms. If what you need is a printed form,
a Microsoft Word document .dot file (which you can construct yourself) might
be the best bet - ask in one of the Word newsgroups.
