Is there a way to get an installation project to create an "All Programs"
menu entry and desktop icon?
 
Never Mind!!!

I had a bad night. I found what I was looking for the moment I opened my
eyes.
 
You can use the File System editor to add files, folders and icons to just
about any place you want on the target system.

If when in the File System editor you do not see those destinations you are
looking for, simply right click on File System on Target Machine, and use the
menu to selected what other folders you would like to see and be able to add
things to.
