Insert supplier name and address into a purchase order

I have designed a purchase order for our school office, but in the supplier
name box our school would like to have the facility of see a drop down table
and choose the name from the list, then it automatically format it into an
address label on the top left corner in the order form. Could this be done in
excel? please help.
 
It may sound straight forward, but I have spent days on how to set the
addresses into separate lines. I have tried importing the supplier name as a
csv file, then filter it, but the address record will just go accross the
page not in a label format.
 
have you tried linking your drop down box to a lookup table to retriev
your data to fill your address field
