Insert item should be accessible from the insert menu in outlook.

If you want to attach a previous email to an outgoing email in Outlook, then
the option is not in the "insert" menu. Instead, it is only accesible from
the paper clip "attach" icon, which is not always first choice. Could this be
looked into?

This is only a problem with Word as the editor. You can switch to using
Outlook as the editor or wait for Outlook 2007, where it is fixed.

The Insert -> Item menu exists in *my* version of Outlook (which is
OL2002). You didn't mention WHICH version of Outlook that you happen to
use.
