How do I create an index based on more than one field?

Thanks or any help.

open your table in design view. open the Indexes window (Indexes button on
toolbar, or View | Indexes on menu bar). enter an IndexName in the first
column. list the field names consecutively (one per row) in the second
column. review/change the Index Properties from the first row of the index
(the row with the index name in it).

Thanks so much, Tina! I was entering a new name in every row so Access must
be treating those a separate indexes? your solution works great!
