Index entry page number with no title

I have a document where I have marked all the headings as index entries. When
I generate the index, several page numbers show up with no associated
heading. I have tried to trace back to see if I have a wayward index marking
but can't identify any and can't see a pattern that will tell me why these
pages are showing up in the index. How can I find and delete these short of
remembering to manually delete them from the index each time I generate it?

rather strange.

Do you have unresolved tracked changes in this document, maybe at/around
stray index entries?

Robert,

Yes. That's it. I left the tracked changes on because I was still routing
the document for review and wanted reviewers to be able to see what had
changed. I saved the document and accepted all the changes, then redid the
index and everything was fine. Thanks again.
