In Word, how do I delete an onscreen email header from document?

I'm working in Word 2003, and a header I've never seen before appeared at the
top of my document. It's set up like a memo header, with lines for "To,"
"CC," "BCC," "Subject," It seems to be attached to the toolbar. I don't know
what I did to make this appear, and I can't figure out how to get rid of it.
Anyone?

You can toggle the email header on or off by clicking on File | Send To
| Mail Recipient, or by clicking on the email button on the Standard
toolbar.
