imports succeed, but can't find data

When I import data from a CSV, map fields and add a few custom fields for the map, then click ok, everything works fine. But there are no accounts actually imported.

I've tried this now on two machines (using same data from csv file) - one is BCM / Outlook 2010, and the other is BCM / Outlook 2013.

I'm at a loss and am looking for hints or clues as to what I'm doing wrong.
