importing excel file to Outlook Contact 2003 into a customized for

I created a form and then published the form. I created an excel file with a
range to import into the customized form. When I imported the file, even
though the new default listed the customized form, the file automatically
loaded the original Contact form.

What am I doing wrong?

You cannot import into a custom form with Outlook's Import Wizard.

As Brian indicated - the Outlook import/export process does not support
custom forms (or any user-defined fields with or without a custom form).

You may find the ContactGenie options of interest - custom form/user-defined
