Icons reset themselves, won't stay where placed and My Docs slow?

Got a customer that we are trying not to do a full reload to but I am out of
"magic" to try so any help would be appreciated.

Anyway here is what is going on.

1. Icons on the desktop seem to rearrange themselves to the left and lined
up. We have turned off autoarragne, etc and turned it on, nothing seems to
make a difference.

2. Related I think is that during open and closing of Explorer all the icons
on the desktop seem to get rewritten. By that we see each one of them reload
from program icon to unknown icon back to program icon. This pretty much
happens any time Explorer is used and I think even IE.

3. In certain applications such as Word when we go to open an existing
document we of course start in My Docs however if we click the drop down to
change current directory we get a complete pause of Word for up to a minute
or more. The drop down does not drop down but just hangs then finally after
a very long pause it releases and we can jump to other directories.

Steps we have taken:

1. Spyware, adware and virus scans. System is clean.
2. Update to latest XP patches.
3. Used that shell tool that will allow you to disable varios shell
extensions. Pretty much disabled everything except for MS entries.
4. Repair install
5. SP upgrades

At each step we got the same results, nothing ever changed. I suspect the
shell is somehow messed up but I have no clue what to try next other than a
reload and yes it would probably be the quicker solution however this is a
small office PC which is critical to daily task so the shell problems have
not reached a point at which downtime would be cheaper than just dealing
with it.

It could be a simple case of a locked desktop in 'active desktop' mode.

rightclick desk, properties, desktop tab, customize button, web tab (uncheck lock and *.htm file selections)

No it was much deeper than that as it effected drop downs in some apps such
as Office and My Docs. We gave up, full reload.
