Icon changing in the start menu

I added an application to my start menu. I later changed the icon from the one I picked when I created it. It sometimes shows the new icon and sometimes shows the old icon. How can I get it to show the new icon consistently

You could try cleaning out the icon cache using a utility such as
Activicons -

To do this in ActivIcons, go to its "Desktop Controls" screen and
increase the icon cache size to 4096. Next, rebuild the icon cache.
The options for rebuilding the cache are on 2 small round buttons at
the lower/right corner. Shut down any programs running in the
background, then click the round button farthest to the lower/right
corner (your system will reboot, and the icon cache will be cleaned
out).

The other possibility I can imagine is if your icon only contains a
single image (most system icons contain several images of different
sizes and color depths). Since Windows picks the image which is
closest to the icon size and colors needed for a particular spot, it
may be picking something more suitable from the original icon. Adding
the other default sizes and colors to your icon might help
