This is a technical newsgroup for questions about Microsoft Access. Did you
look at the other topics in here before posting? None of them pertain to
filling out a VISA application.

I doubt anyone will be able to advise you in a newsgroup. Go get the form
and fill it out. If there are specific fields that you don't understand,
then someone might be able to help you, but again, not in here.

I would think the office where you pick up the form would also be willing to
help you complete it.

I would guess you should contact the embassy of the country for which
you want the visa and ask them. I would guess they will have a form and
some instructions.
