I have multiple contacts in my Outlook address book

When I am sending an email and I go to select a "To", I pull down the box
"Show names from" and there are four listings for Contacts. Two of the
Contacts have names from my Contact folder and two have no names. Some of my
contacts are only in one listing. How do I get all my email addresses to show
under one, and, only one, listing?

Remove the Outlook Address Book service from Outlook, stop and restart it,
add the service back in, and stop and restart again.
