We use one at work. You don't need a template. All you need is a date, payee
name, account number, amount, maybe a few lines for explanations or
whatever, and signature lines for the person making the request and whoever
needs to authorize the request. I'd also suggest a space for an invoice
number if available and a due date but you may not need those.
And don't forget to add your logo! That turns it into a professional looking
request form instead of something you've slapped together in 20 minutes
(even tho you can do it in 20 minutes.)
