When setting up Active Directory Integrated Zone - does
DNS service need to be installed and configured on each 2K
server (domain controller)? Or does it only need to be
installed on 1 domain controller?

Im assuming it will need to be on all servers acting as
domain controllers just in case one fails - the other
could pick up?

AD Integrated DNS must be on Domain Controllers.
AD Integrated DNS is replicated with the rest of the domain replication info
to all DCs with DNS installed.
You don't need DNS on ALL the domain controllers, but it does no good (from
a redundancy point of view) to put AD Integrated DNS on one DC either.
Set up more than 1 DC as AD Integrated DNS.

DNS should be installed on at least one DC, preferably two but, the more the
merrier.
If you have two fine make sure all clients point to both. The first DC you
have should point to its own address, the second should point to the first
as primary the to itself secondary.
If you have three the Second and the third should point to the first DC as
primary then to the second or third as secondary and so on on all DCs.

It is recomneded that all DCs point to the first DC in the domain as primary
because it will be the Infrastructure Master. Which brings up another
subject, Global Catalog servers, If you have only two DCs you should make
the second DC the Global Catalog Server, You can have as many GC servers as
you want but it is recommended that the GC *not* be on the Infrastructure
Master but you must have at least one GC server.
