How to set up a tick box in Word for surveying emailed responses?

I've written a survey in Word 2007and I want to email it to a large group of
students to get their ticked responses to specific questions. I remember some
years ago creating something similar but I don't know how to do it in Word
2007. It's for ease of response for the students. I want them to then send
the document back ticked appropriately.
Can you help?

Doug Robbins - Word MVP

If you do not have the Developer tab displayed, from the Office Button,
select Word Options and then from the Popular Options, check the Show
Developer tab in ribbon and then on that tab, from the Controls section of
the ribbon, click on the Legacy Controls item (it has an icon that includes
a hammer and a spanner). Then, you can use either a FormField check box
(which requires that the document be protected for filling in forms) or an
ActiveX control check box.
