How to set info --date, name without underline disappearing?

I want the top of a page to have headings for info that can easily be changed
without the underline disappearing -- i.e. Date Name

You don't provide us with a lot of information to even guess what the
problem may be. Is this a form you are working with or trying to setup?

You could (also) simply insert a table with as many rows/columns needed
to separate the info you want to display here. Each cell contains one
info (Date, Name, etc.). Make sure you apply a paragraph style to the
table that is set to underlined font formatting.
