How to send form results to a database and generate a formated ema

I have created a FP2003 form which will post to an access db upon SUBMIT. I
would like to also send form results in a formated email upon the initial
submission.

The bottom line that I am trying to accomplish is to have an employment
application sent to a single email address in a nice formated manner -
something that includes the original question as well as the form results for
that question. The DB is optional for my requirements if there is an easier
way to do this. I'm a novice at web design but no stranger to programming.
