How to make form a template? How to enable updating of form and ta

I am trying to design a database to calculate nutrients a patient receives
from their intravenous (IV) nutrition and/or formulas they receive. I have
designed a form to enter patient admit data and another form to enter
information on the volumes of the IV nutrition and formula they receive. I
was hoping the form to enter volumes of the IV nutrition and formula would
work as a template, I would pull up the patient's name from the patients
listed in the table and then enter the patients new weight measurement and
volumes of whichever products they receive. However, the form I developed is
not working like a template, but has a new record for each of the patients.
The user would have to look though all the records to find the patient that
they are wanting to work with. Is there a way to change this form to act
like a template and to use this template to update my patient table?

Also, in playing with this form I am finding that some of the text boxes do
not allow me to change, or update, the patients information (in this case,
their weights). How can I enable these text boxes to change and to update
the patient table based on what is typed into the text box? I am using
Access 97 at work.

I see that in an earlier working version of this database I was able to
update the table using the form. As I have not yet learned how to save a
back-up version of an Access database to my computer at work, I have e-mailed
the database, as an attachment, to my home e-mail address. Then if I mess it
up during the day, I have an earlier (pre-mess-up) version that I can e-mail
back to myself at work. There must be an easier way!
