how to generate a form weekly

I have a form in excel...and i want to link it to access so i can generate
the form weekly per employee as a report?importing it didint work.can someone
help..

Are you talking of an Excel data entry form or a fill in form for output such
as an invoice? Are you wanting the form to get its data from Access?
If it is the latter, highlight the form in Excel (if it is protected,
unprotect it). Copy it. Open Access and create a new report on the
table/query in design view. Select the detail header and Paste. Insert your
fields where you want them on top of this form.
