how to create and save a custom name tag in word

I am attempting to create and save a boilerplate in Word2003 for producing
nametags (Avery 5392) They are to contain a graphic and header of the
organizations name. This is a very simple process in WordPerfect (my word
processor of choice) but I have yet to figure out how it can be done in Word.
I have gone through the Word Help and How-to to no avail. If I didn't need
this for another user I would revert to the tried and true..

Use the New Document button in the Labels dialog. Save the document as a
template.
