How to create a field w ith a blocks to enter each letter IN WORD

In Microsoft Word i want to create a form which consists of a fields which
allows the user to enter each letter in a rectangle blocks. This is same as
we fill up the form like name, we write it as m a n s o o r

This will be quite fiddly to achieve, requiring the use of a table with a
form field in each cell set to accommodate only 1 letter. You will have to
juggle the cell margins and the line spacing to get a sensible result.
