How to associate icon with extension?

Does anybody know how to associate an icon with a file extension that
windows is not familiar with?

There is no association, other that (1) an icon exists inside an executable,
or (2) An association is made via an installation utility which adds a
registry entry (ruled out by you), or (3) a user associates an icon with a
shortcut.

Thanks Kevin,
it seems that the best way to go, is to use the installation utility
that comes with VS 2003 or 2005. i tried that successfully.
