How to add a checkbox that can be checked and unchecked in word

I want to add a simple checkbox that the user can click with the mouse, which
will show alternatively and checkmark and then blank. Please help. I am
using Word 2007, and I have already added a checkbox with the Developer tab,
design mode, etc, but the box will not allow the mouse click and add the
checkmark. Please advise.

Presumably you are in 'design' mode if the Active X check box doesn't work -
