How do you scan a prepared document or form and type in your info

How do you scanned an already printed form and then fill in the blanks from
office word?
 
You could either run the file through an OCR program or paste the
image into Word and put text boxes where you wish to type, make the
text boxes transparent with no borders, for a one off form this is
probably the easiest and quickest way.
