Right click somewhere up on the tool bar area, and select "Forms". That
will bring up another tool bar which allows you to select various
controls including a "Button" and an "Option Button" one of which is
perhaps what you're asking for. Select that, and place the button where
you want it.
Right clicking on your newly created button allows you to assign a macro
to it and so forth.
