How do I save pages within a form

We are using forms as an initiation point for our scheduling system, which is
mainly manual. I am trying to modify the form and having problems.
Basically the form has a couple of pages, which the sales people must
complete to book a consultant. This form is then put into the consultants
calendar entry.
The problem is that the pages are visible while the form is passed around
and authorised, but whenever it is put into a calendar entry the pages are
lost.
This used to work but now whenever I modify or create a new form it fails.
I have recently been upgraded to Outlook 2007, but was also having the
problem on Outlook 2003.

How can I get teh whole form, including all pages, to be saved and visible.

You must publish the form either to the Organizational Forms library or to every person's Personal Forms library, with the "Send form definition with item" box unchecked.
