how do I save a document as a template to use over and over

I made this document A draw form for my business The information on it will
change everytime I use it but the document itself needs to stay the same. How
can I save it and still be able to write over it in word. Everything I try
erases the original document when I start to type on it.

In the document type, select Word document template.
