How do I move all Outlook contents from old computer to new one?

I use Outlook 2003 on my old desktop running XP Home and having just
purchased a new desktop running Vista, want to move all of my messages and
contacts to the new one. I've looked at importing tips but can't find
anything that helps me with my situation. I've already installed Outlook 2003
on the new pc but would be ok with just using Windows Mail instead. What I
need is step-by-step instructions that my tiny brain can understand. Can
anyone help?
