how do I modify context menus

How do I modify the context menus I get when right clicking?

For example, if I right click in a folder window and select "new" I'm
presented with a choice of file types. I'd like to delete most of these
and add some that I actually use. Can I do this?

Similarly, if I right click a file and then select 'open with', I'd
like to change what shows up.

Some of the choices I now have date back to when I first got the system
from all that unsolicited (by me) pre-installed software.
