How do I map custom fields effectively?

I have designed and "published" a custom form and would like to use it to
transfer data from an Access file to a Contacts folder [filling in the fields
on the new form]. I have also made it the "default" form and it shows up when
I click for a new contact. I would like to import Access files into this new
"form". When I go to Import a file I ultimately end up on the Map Custom
Fields screen. When I go to the Map Custom Fields screen I cannot seem to
move unused fields from the right [MS Outlook Contacts] to the left - in
spite of the instructions on the top of the field that say I can. Nor can I
move "values" from the Access file [in the left box] to the right box unless
I put them in the "mapped from" column. I'd just like to have the values from
the left go into the fields [I created in my custom form] in the right box.
Except that the fields from ym custom form are not showing up. What do I need
to do to rectify this problem?

Outlook doesn't support importing to custom fields. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
