If it is to be printed and filled out you may want to use a Word document
for your form, in which case you would do better to check with a Microsoft
Word newsgroup. This newsgroup is for Microsoft Access, a relational
database program. I should tell you that no matter the newsgroup, you will
have to be more specific than you have been so far about your needs.
