How do I make index entries hyperlink to the body text?

Re Word 2003 Index

I would like my index entries to be hyperlinked to the body text. i.e. the
same way you can make a TOC hyperlinked. Ultimately I'll be exporting to PDF
so I'd like the links to survive Adobe PDF Maker 7.0.5.
